Position Overview: Case Managers care for over 2,500 clients and execute nearly 15,000 home visits a year. They are responsible for activities such as intakes, recertifications, reassurance visits, delivering meals and other activities and services as needed. Paid As: Hourly, Non-Exempt Full-Time EmployeeHours of Operation: 8:00 AM - 4:30 PM Monday - FridayReports To: Client Services SupervisorOther Relationships: Works with Operations/Finance Department, Nutrition Department, Marketing Department, Meals On Wheels Board Members, Community Volunteers, and Meals On Wheels clientele.AGENCY EXPECTATION OF EMPLOYEEMaintain confidentiality of client, donor, volunteer, staff and board informationSupport the mission, vision and goals of Meals On Wheels, Inc. of Tarrant CountyRepresent Meals On Wheels in a professional manner at all timesMust have dependable transportation, valid driver license and personal vehicle insurance as required by the State of TexasRequired to pass criminal background check and have a free and clear MVRAssist with meal delivery when volunteer callouts make it necessary for staff to deliverContribute to other projects when necessaryESSENTIAL JOB TASKSPerform intakes on prospective clientsDeliver meals to clients (this is often done several times a week)Perform triannual recertification visitsExecute reassurance visits as neededDeliver items such as wheelchairs, rollators, walkers, canes, shower chairs, supplemental food, toiletries, and other helpful resources that benefit clientsPerform well-checks on clients that may have fallen or are injuredMust possess customer service skills such as being a team player, having a positive attitude, a professional demeanor, and the ability to maintain confidentialityDetail-orientated with computer proficiency and oral and written communication skillsReliable and punctual with dedicated professionalism to position and dutiesMust be able to work independently and proactivelyWork on special projects with area entities and organizations on collaborations and grantsOther duties as assigned by VP Client ServicesQUALIFICATIONSFour-year university degree in Social Work, Human Services, Psychology, Sociology, or related fieldREQUIREMENTSPhysically able to lift and carry objects of 25 poundsPhysically able to enter/exit your vehicle multiple times a day, walk up and down stairs, reach, stretch, bend, and sit for multiple hours at a time EXPERIENCEExperience in social work or case management preferredCompensation details: 18-20 Hourly WagePI6b8b88d9acc4-25660-37264325
